Form 4: Nomination Paper and Acceptance Form
Each office has a distinct form that must be used and each requires a different number of signatures from eligible voters:
- Edmonton Catholic School Division Trustee - 25 signatures needed
Once completed, Form 4 must be signed by a Commissioner for Oaths. If candidates submit their nomination package in person, Edmonton Elections can provide this service free of charge.
Form 5: Candidate Financial Information
Candidates must inform Edmonton Elections of any changes to the information provided in the Candidate Financial Information form within 48 hours. Visit Campaign Financing to learn more about financing requirements.

Criminal Record Check and Vulnerable Sector Screening
As part of the nomination process, Edmonton Catholic Schools requires all Trustee candidates to submit a Criminal Record Check and a Vulnerable Sector Check, both conducted by the Edmonton Police Service. These checks must be completed no later than 45 days before submitting a nomination package. All associated costs are the responsibility of the candidate.
Criminal Record Check
Visit the Police Information Checks Office, at #108, 14315–118 Avenue (Nexus Business Park) to complete the criminal record check.
Vulnerable Sector Check
Once Edmonton Elections has confirmed your notice of intent has been received, contact the Edmonton Catholic School Division Board Office to schedule an appointment to acquire a Vulnerable Sector Check.
